Phoenix Project
is one of a number of services provided

by Hammersmith
and Fulham Action for Disability, HAFAD
is an organisation of disabled people whose work is based on the
principles off self-determination and choice. Phoenix exists to make a
difference in an active and positive way.
Who can the Phoenix Project benefit?
The Phoenix Project works with young disabled adults, who want to work towards greater independence and can participate in setting the goals they wish to achieve.
What is meant by greater independence?
Independence means different things to different people. For example it could mean moving into fully independent or supported accommodation;learning skills that will lead to work; practising self care or developing skills.
How we work
Phoenix participants are allocated a personal project worker and together they will draw up an individual work programme based on their long and short term goals.
Each person's progress is monitored and regular contact is maintained with all involved including tutors, occupational therapist, social workers, parents and carers.
Phoenix runs on an academic year basis and offers a flexible timetable and individual activities which is developed and adapted according to the needs of members.
We offer a fast track to greater independence. Everyone receives maximum assistance to achieve there goals as quickly as possible. Once goals are realised members will be supported to move on from Phoenix.
Phoenix
activities change of frequent basis, and all participants have an individual
timetable, usually with different days of attendance.
Some activities
take place on site, some in members' homes and many are undertaken
in the wider community.
personal development/ assertiveness/confidence building: development of social skills and friendship building.
travel training : learn and practice travelling independently to community education classes, work placements, friends and relations' homes and local amenities.
independent living skills: including for example self care and hygiene, safety, budgeting, shopping, cooking etc.
fitness: includes input from physiotherapists and occupational therapists to devise suitable individual fitness programmes using community facilities.
computer skills: word processing, access to the internet, arts packages and educational software.
Joining Phoenix
Individuals may refer themselves or a referral can be made on their behalf.
Applicants should have an allocated a social worker to help the referral and subsequent placement and to monitor the quality of service provided by the project.
A trial week will be offered so that the individual can experience the project first hand. Project staff will also be able to access whether the Phoenix project is suitable.Once a place has been offered and accepted and funding agreed a project worker will discuss the aims and agree a work programme.
Phoenix members have easy access to a number of HAFAD's services: advocacy, counselling, welfare benefits advice, an independent living, employment and housing advice which can be vital.
